Affonso recently featured for Cerro Largo in his native Uruguay before agreeing to his new contract with Juan Pablo II, which will run until 31 December 2025.
In a statement on Instagram, the Peruvian club wrote: "Club Juan Pablo II welcomes Uruguayan striker Mauricio Affonso, who returns to Peruvian football with his goal-scoring instinct and vast experience to wear our team's shirt.
"Let’s go, Mauricio!"
Juan Pablo II is the 10th club of Affonso’s career, having previously played in Argentina, Peru, Saudi Arabia, South Africa, and for several teams in Uruguay.
During his time at Sundowns, Affonso made 22 appearances and scored six goals.
He also won two South African league titles and the 2019 Telkom Knockout – the final edition of the tournament before it was replaced by the Carling Knockout Cup.
