Madigage, who was appointed Gordon Igesund's right-hand-man in July, was driving alone when he hit a donkey and lost control of his car.
The sad news was confirmed by Limpopo provincial police spokesman Brigadier Hangwani Mulaudzi.
"We can confirm the unfortunate death of Mr Thomas Madigage. He was involved in an accident yesterday so as we speak now the family has already been notified," Hangwani confirms.
"He was travelling from Burgersfort to Polokwane. We can confirm that a donkey was involved and the car rolled. He was certified dead at the scene," Hangwani says.
In a statement, Safa president Kirsten Nematandani said: "We are shocked and at a loss for words and, as we speak, we are on our way to meet the family and find out what actually happened.
"We would like to pass our sincere condolences to the family, friends and the entire nation on this shocking development."
