Jomo Cosmos edged AmaZulu 3-2 on penalties to secure the final Nedbank Cup last-16 berth on Tuesday night.
The match was decided from the spot after 120 minutes of football produced a 1-1 stalemate in Potchefstroom.
Denver Mukamba drew first blood for Cosmos with a left-footed strike midway through the first half.
Usuthu suffered a further seback when their captain on the night Marc van Heerden went off with an injury just before the interval, with Tshepo Liphoko taking his place at Olen Park Stadium.
The NFD side, however, equalised in the 72nd minute through Zambian midfielder Allan Mukuka.
Steve Barker's outfit were dealt another blow in extra time when striker Mame Niang was sent off. Niang was dismissed in the 106th minute for dissent after appealing for a penalty.
Cosmos then prevailed in the shootout, and will find out their next opponents in the event on Thursday when the PSL holds the last-16 draw at the Carlton Centre in Johannesburg.
Cup holders Mamelodi Sundowns, SuperSport United, Polokwane City, Mpumalanga Black Aces, Baroka FC, Mbombela United, Golden Arrows, University of Pretoria, Free State Stars, Orlando Pirates, Bidvest Wits, Platinum Stars, Santos, Maritzburg United and Magesi FC await in the next round.
