Fans of Heartland watched their team surrender a two-goal lead to lose 2-3 at home to Enyimba, then took their anger out on referee Gabriel Adigwe.
The match official was brutally assaulted before he was rescued by security officials, after which the fans turned on each other.
Enyimba were not spared in the violence, as Team Manager Deboy Enwuronwu was also assaulted.
NPL's Acting Executive Secretary Tunji Babalola says the Owerri club can expect disciplinary action
"These are the kinds of things we don't want to see in the league. Both sides played good football and the fans should have accepted the result rather than turn violent.
"We will take action," he said.

Espanyol climbed into the top half of the Spanish Primera Liga standings after a hat-trick by midfielder Kalu Uche helped them to a 5:1 thrashing of the visiting Rayo Vallecano on Sunday.
The Nigeria international scored twice in the first half either side of a Philip Coutinho brace as the Catalans raced to a four nil lead at the break.
Rayo pulled one back through Raul Tamudo in the 54th minute, before Uche capped the win on 69 minutes.
Uche, who rejoined Espanyol in the winter transfer window after an earlier spell in the 2000-01 season, has now scored four goals in five appearances for the club.
Meanwhile younger brother Ikechukwu Uche was on the losing side after playing all 90 minutes for Granada in their 2:0 loss at Atletico Madrid today.
